About Anal Fissure
A fissure or an anal fissure is a painful condition caused by a small cut or tear in the moist tissue lining of the anal canal. Fissures are common, and cause a lot of discomfort, but do not cause any serious problems. Although, it can occur at any age, it is more common in young infants. In most cases, fissures are confused with other anal problems, such as piles.
Causes of Fissures
Fissures usually occur when there is a trauma to the inner lining of the lower rectum caused by the passage of hard or large stools during a bowel movement. Some other common causes of fissures include:
- Constipation
- Straining during bowel movement
- Crohn’s disease
- Chronic diarrhea
- Childbirth
- Anal intercourse
Rarely, anal cancer, tuberculosis, syphilis, herpes, and HIV may also cause anal fissures.
Signs and symptoms
The following are the common signs and symptoms of anal fissures:
- Severe pain during and after a bowel movement
- Blood in stool
- A visible cut around the anus
- Itching or irritation around the anal region
Diagnosis
In addition to reviewing the medical history and symptoms of the patient, a physical examination is done to check the anus and the adjoining areas. Further, a rectal examination is done to confirm the diagnosis. If the doctor suspects the fissure to be a sign of any underlying diseases, then further tests, such as anoscopy, flexible sigmoidoscopy, and colonoscopy are recommended.
Treatment
Fissures usually heal within a few weeks with certain dietary changes, such as, increasing the intake of fibers and water. Having fiber-rich foods and drinking enough water helps in softening the stools, preventing any injury while passing stools, and thereby promoting healing. Additionally, over-the-counter stool softeners and topical painkillers can be used to relieve pain. If symptoms do not subside within two weeks with these measures, then the person must consult a doctor.
Surgery
Surgery is recommended in case of severe and chronic cases. Lateral internal sphincterotomy (LIS) is a common surgical procedure, wherein a small incision is made in the internal anal sphincter muscle to reduce muscle spasm and pain.
Although conventional surgery is still used, laser treatment for fissure is the most preferred option.
Laser treatment for fissure
In this procedure, an infrared radiation beam is passed near the affected region to cut the fissure and relieve spasm, thereby facilitating the tissue to heal.
Advantages of laser treatment for fissure over conventional surgery
Laser treatment has the following benefits:
- Out-patient procedure
- Non-invasive procedure
- No or minimal blood loss
- No or minimal risk of damage to the surrounding tissue
- Minimal pain
- Quicker recovery
- Minimal risk of post-surgical complications
